- Taking the time to become familiar with some of the local senior services prior to having any need, can help decrease your stress when it is necessary to use the services!
- This course has been organized so you have valuable face to face interactions with providers allowing you to become educated on their services. Through this education you can become a better patient advocate!
Between June 1, 2010 and September 30, 2010 putt on the various greens established at participating organizations and get your scorecard validated.
- Maintain control over your healthcare choices throughout the aging process by establishing relationships with various professionals who can help. Learn about what questions to ask!
- Discover how individuals move between services within the healthcare continuum.
Become eligible to win either $1,000 or $500! No purchase necessary.
- Get your scorecard validated at each service location, drop it off at The Erie Sports Store by Oct. 4th 2010 and be invited in to the LIVE DRAWING in October where winners will be announced! We wish that you are at least 40 years of age to participate in this senior services promotion.
